- Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and subpart 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ). This is a 100% Small Business Set-Aside. The Bay Pines VA Healthcare System in Bay Pines, FL intends to Purchase the following plumbing supplies: P/N DISCRIPTION QTY U/I PRICE NH-2 1. 2" CHARLOTTE NO-HUB CI PIPE 9000 FT NH-2 2. 4" CHARLOTTE NO-HUB CI PIPE 1000 FT NH-16 3. 2" NH SHORT SWEEP - DOMESTIC 800 FT NH-22 4. 2" DOMESTIC NH COMBO 800 FT NH-12 5. 2" DOMESTIC NH 1/8 BEND 400 FT NH-14 6. 2" DOMESTIC NH 1/16 BEND 200 FT NH-42 7. 2" DOMESTIC NH P TRAP 100 EA 270-229 8. 2" DOMESTIC "GRIPPER PLUG" 100 EA 270-237 9. 3" DOMESTIC "GRIPPER PLUG" 50 EA 270-245 10. 4" DOMESTIC "GRIPPER PLUG" 50 EA 2 PFNHCK 11. DOMESTIC NO-HUB COUPLING 2000 EA 4 PFNHCP 12 DOMESTIC NO-HUB COUPLING 500 EA This solicitation will result in a FIRM FIXED PRICE type contract. Delivery will be FOB Destination prices. The delivery will be for all material to be delivered on or about 30 days after receipt of order; to VA Medical Center Bldg 100/warehouse 10000 Bay Pines Blvd Bay Pines, Fl 33744. The RFQ number VA-248-08-RQ-0149 shall be clearly identified on all submitted materials. The corresponding NAICS code is 334516; size standard is 500 the following provisions apply to this acquisition: 52.212-1 Instruction to Offerors; 52.212-2 Evaluation of Commercial Items. The specific evaluation criteria included in paragraph (a) are: 1) price 2) technical capability 3) past performance - past performance and technical capability when combined are equal in importance to price. Please submit 2 references with quote; all offerrors are cautioned that if selected for award, they must be registered with the Central Contracting Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov. Confirmation of CCR registration with the following criteria: Technical Capability (compliance with the requirement s of FAR Clause 52.211-6 Products) and Price. The provision can be downloaded by accessing the website listed in this solicitation. The following clauses also apply to this acquisition: 52.212-4 Contract Terms and Conditions.
