- This is amendment 05: a. This amendment will serve as a notification that this solicitation has been amended to reflect the questions and answers submitted as of 10 January 2008. To retrieve these you must be certified through DLIS. The FT P website has been updated with this amendment and questions. b. This posting will change Amendment 03. The correct web address for DLIS is www.dlis.dla.mil/jcp . The contractor will click on DOCUMENTS to find a blank DD Form 2345. Complete the DD Form 2345 (preferably typed) and supply the additional documentation which must be mail to DLIS for processing. The correct address for DLIS is at the top of the DD Form 2345.
