Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation VA-256-08-RP-0107 is issued as a 100% Small Business Set-Aside Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-23. No telephone offer will be accepted. Only written offers received directly from the requester are acceptable. The North American Industry Classification System (NAICS) Code 811219. This size standard is $6.5 Million. OFFERORS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. BRAND NAME OR EQUAL ITEMS WILL BE CONSIDERED; IF AN "OR EQUAL" ITEM IS OFFERED, DESCRIPTIVE LITERATURE SHALL BE PROVIDED. Item 0001 - Replacement of Dust Capture Equipment and Duct Lines (See Statement of Work) - 1 Job = $______________ FOB: DESTINATION ONLY. Offeror proposed Period of Performance is __________ calendar days after receipt of award. FAR 52.252-2, Clauses Incorporated by Reference (1998) - incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Incorporated herein by reference the following FAR Clauses and Provisions apply to this acquisition: 52.211-6, Name Brand or Equal (AUG 1999); 52.212-1, Instructions to Offerors - Commercial Items (NOV 2007) - offerors must comply with all instructions contained in this provision; 52.212-2, Evaluation - Commercial Item (JAN 1999) - The Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technically acceptable (2) Price and (3) Delivery. Technically acceptable is more important than price and delivery. To be technically acceptable the contractor must provide knowledge of installation of dust capture systems (references may be requested). Delivery will be considered as of equal importance with price. Price will be evaluated to determine its fairness, completeness and reasonableness as it relates to services offered. 52.212-3, Offeror Representations and Certifications - Commercial Items (NOV 2007) - a completed copy of this provision shall be submitted with the offer; 52.212-4, Contract terms and Conditions - Commercial Items (Feb 2007); 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (DEC 2007) [(b)(5), (15), (16), (17), (18), (19), (20), (21), (22), (24(i)), (27,),(28(ii)) (30); (c)(1), (2) and (4)]; 52.215-5, Facsimile Proposals (OCT 1997), 52.225-18, Place of Manufacture (SEP 2006). VA Acquisition Regulation (VAAR) 852.252-1, Provisions or Clauses That Require Completion by The Offeror or Prospective Contractor (DEC 1999), 852-211-70, Service Data Manual (NOV 1984). The contractor agrees to furnish one copy of a manual, handbook or brochure containing operating, installation, and maintenance instructions (including pictures or illustrations, schematics, and complete repair/test guides as necessary). Where applicable, it will include electrical data and connection diagrams for all utilities. The instructions shall also contain a complete list of all replaceable parts showing part number, name and quantity required; 852-211-71, Guarantee (NOV 1984) - The contractor guarantees the equipment against defective material, workmanship and performance for a period of said guarantee to from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material which are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor; 852.211-77, Brand Name or Equal (NOV 1984) 852.270-4, Commercial Advertising (NOV 1984); 852.273-70, Late Offers (JAN 2003), Electronic Invoice Submission. STATEMENT OF WORK, REPLACEMENT OF DUST CAPTURE EQUIPMENT AND DUCT LINES Contractor is to provide and install a new dust capture system, Grizzly brand or equal, correctly sized for all equipment in the Engineering Service Carpenter Shop. Equipment and ductwork sizing and installation shall be in accordance with the equipment manufacturer's recommendations. Contractor shall provide all labor, materials, tools, supervision and equipment to completely remove the old dust capturing system and install the new dust capturing system located in the Carpenter Shop, Building 2, Room 08, Overton Brooks VA Medical Center, Shreveport LA. Contractor will visit site to verify conditions as they exist. Equipment Removal: 1. Disconnect and remove the present dust capture system. 2. Disconnect and removal all the present duct and trunk lines. 3. Disconnect and remove all of the present hangers for the duct and trunk lines. 4. Remove and properly dispose of all items associated with removal of the existing dust capture system. Evaluation, Installation and Testing: 1. Provide and install a new dust capture system, Grizzly brand or equal; correctly sized for all permanently installed dust generating equipment located in the Engineering Service Carpenter Shop. Sizing and equipment installation shall be in accordance with manufacturer's recommendations. 2. Provide and install new duct and trunk lines, Nordfab Quick-Fit brand or equal correctly sized to match the new dust capture system and shop equipment. 3. Core drill new penetrations in concrete floor required for additional equipment hook-up. Seal all new penetrations as required. 4. Evaluate the entire carpenter shop equipment hook-up for proper duct and trunk line lengths and diameters. There will be a total of 5 floor penetrations and 7 pieces of equipment hook-ups. (1 piece of equipment requires a top and bottom hook-up for a total of 8 hook-ups.) 5. Evaluate present floor penetrations for proper diameters and enlarge as required for new dust capture system. 6. Evaluate and install new duct and trunk line hangers and supports. 7. Install automatic blast gates for each piece of equipment (total of 7 blast gates). 8. Test system for proper operation. 9. Contractor shall visit the site, prepare and submit shop drawings of the new system for approval by Engineering Service representative. 10. Contractor shall be responsible for removal of all debris from station and repair of any damages caused as a result of the removal of the old dust capture system and installation of the new dust capture system. 11. All work shall be accomplished between the hours of 7 AM to 4:30 PM, Monday thru Friday, excluding federal holidays. Work shall be coordinated with Engineering Service representative. 12. Connect new system to existing electrical service, provide disconnects, switches, etc. in accordance with the National Electric Code (NEC). If the new system requires additional electrical service or wiring, the contractor shall furnish and install additional electrical service as required by the manufacturer. All wiring shall be a minimum #12 THHN solid conductor. All interior conduit shall be ??" EMT with compression type, insulated throat connectors. Exterior conduit shall be ??" minimum rigid or seal tight conduit. New service shall include all disconnects, switches, etc. in accordance with the NEC.
