- The contractor shall provide Critical Test Results Management Service, a hosted, managed software service which assists user to communicate stat, abnormal or critical patient information within a multi-model environment to the referring physician community. Services include Radiology, Laboratory, Pathology and Cardiology. The services will be at the VAMC, 800 Zorn Avenue, Louisville, KY 40206 for a base period of 10/01/07 - 09/30/08 with two (2) one-year option to renew. This is a Total Small Business Set Aside, Commercial Item procurement. The following clauses and provisions plus any addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52-212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes or Executive Orders. The solicitation package will be available for downloading o/a 9/5/07. No web based offers will be accepted.
