- The New Jersey National Guard (NJARNG), US Property & Fiscal Office New Jersey Contracting Office intends to establish one Blanket Purchase Agreement to provide the following Mobile Dental and Medical Services at multiple locations throughout the s tate of New Jersey. Multiple awards are not anticipated. The anticipated Agreement is for one (1) Base year and four (4) option years. Offerors must be able to provide both mobile medical AND dental services, for multiple locations simultaneously, to inc lude all management, labor, material, equipment, certifications and supplies necessary to provide medical and dental services at various armories and training locations throughout New Jersey. Medical services will be performed by licensed medical professi onals with oversight by the NJARNG, Office of the State Army Surgeon. ALL QUESTIONS PERTAINING TO THIS SOLICITATION ARE TO BE SUBMITTED IN WRITING to Kathy.m.hines@us.army.mil AND lisa.t.hall@us.army.mil . Prospective Offerors shall submit one copy of their proposal. Proposals can be hand delivered, faxed, or e-mailed. All materials must clearly identify the solicitation number W912KN-08-R-0001 for identification purposes. Responses to this offer must be received by the designated individual no later than 11 January 2008 at 4:00 p.m. Submissions after this time will not be accepted. Offerors are cautioned to submit enough information to ena ble the Government to fully ascertain each offerors capability to perform all of the requirements of this solicitation as per the Statement of Work (SOW). The data submitted with each proposal should be complete and concise, failure to submit the require d information may preclude consideration of the offer for award. Selection Criteria to arrive at a Best Value decision will be based on technical and price equally.
