- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W9124V-08-T-0001 and is issued as a Request for Quote (RFQ). The NAICS code is 423450 and the size standard is less than 100 Employees. The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular 2005-21, Effective 07 Dec 2007& Class Deviation 2005-o0001. The proposed contract is 100 percent set aside for small business concerns. The acquisition is a firm fixed priced purchase order in support of the Montana Army National Guards J 5/7. The request for quote is for medical supplies and goods. This is in support of soldiers training in Jan. THE DELIVERY DATE HAS TO BE JAN 25. THERE WILL BE NO EXCEPTI ONS TO THIS DATE. When submitting a quote to the government, include the last date the purchase order can be received to meet the above delivery date because there will be no exceptions to this. Funds are subject to the availability of funding by the gov ernment but will be in place before award. The items intended for purchase: See Attached List on the website www.montanaguard.com/uspfo/html/simple_acquisitions.cfm. If the offeror is quoting an equal item, please provide technical specification and warr anty information to insure all items are compatible. The provision of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Nov 2007) applies to this RFQ. 52.212-3, the clause needs to be completed and a signed copy of this provisio n shall be submitted with any offer. 52.212-2, Evaluation, Commercial Items (Jan 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Price, Technical Capability, and Past Performance. Past performance and technical capability, when combined, are less important when compared to price or cost. Vendors are requested to submit past performance information to include persons of contact on their last two contracts for similar products.
