Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information ncluded in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Missouri Army National Guard has requirements for PHA and SRP events 5-6 January and 12-13 January 2008 at five (5) locations throughout the state of Missouri. Please see attached sta tement of work for each location: The quantities and locations are as follow: CLIN 0001 - 100 persons, Jefferson City, MO; CLIN 0002 - 300 persons, St. Joseph, MO; CLIN 0003- 160 persons, Cape Girardeau, MO; CLIN 0004- 400 persons, Springfield, MO; CLIN 0005 - 23CLIN 0001 Contractor will provide a price for minimum delivery quantities for each of the services listed. Contractor will provide incremental unit pricing for additional quantities up to 100 pax for each service listed. Contractor will pro vide event management services to ensure that all services are delivered in an efficient manner and meet the Commanders time schedule. Immunizations (Select Soldiers) a. Needles b. Hep A/B, Tetanus/Diphtheria, Flu shot (will only need personnel to administer, vaccine will be supplied by the National Guard.) * Record date of administration on SF 601 and if available DD Form 2766 6. Dental Exam (Digital Machine, separate techni cian for each machine provided) * Bring enough personnel and equipment to run two teams a. Dental Exam (All Soldiers) c. Bite Wings (Select Soldiers) d. Complete a DA 5570, SF 603 to include classification, PSA, and Risk Factors for each serv ice member examined. Enter DD2813 into Dental Record if furnished by the service member. e. Enter examination results into Denclass. Upload panograph and bitewings performed to Denclass f. Ensure dental record has been initiated to include DA Fo rm 5570, SF 603 and or DD 2813, Pano in record that is of adequate quality for diagnostic purposes g. Past Performance shall be evaluated based upon available information and list of three (3) references of previously contracted similar services to include the DOD agency name, address and telephone number. Provided the name of an individual that has direct knowledge of your performance as well as the Contracting Officer name and tel ephone number. Pricing to include multiple award evaluation and economic preferences identified.Technical and past performance, when combined, are equal to price in determining the source selected to perform the services. The Government intends to award the contract without conducting discussion unless the Contracting Officer determines discussions to be necessary.
