Description
Services required of an Architect or Architect/Engineering firm to design, furnish plans, specifications, and construction period services for Project 621-08-101, Correction of Risks Identified from Inpatient Risk Assessment, Building 200, wing E2, James H. Quillen VA Medical Center, Mountain Home (Johnson City), TN. Work includes preliminary and final contract drawings, cost estimates, specifications, and construction period services. A/E required to develop Deductive Bid Alternates as necessary to assure award of contract within available funds. This project is located at building 200 in wing E2, inpatient mental health, Psychiatry department. The department of inpatient mental health has done a risk assessment evaluation of there area and this project is to correct the problems found involving the safety of the patients and stall as well. The project will consist of minor demolition and some extensive installation. There will be replacement of fluorescent lighting with recessed incandescent lighting above the bed area and restrooms. Any drop ceiling needs to be replaced with a solid ceiling. Installation of new corridor doors with three point hinges, and in the patients rooms new angled top doors with piano hinges need to be installed. Self closing automatic locking doors to be installed on all staff offices and a fixed wall with viewing glass surrounding nurse's station. All wall edges and corners to be covered with corner guards, remove all wall tile and replace with Avonite solid surface wall panels. The existing thermostats mounted on the wall will be replaced with a computer controlled duct mounted thermostat eliminating the existing exposed thermostat box. All counter top basins need to be enclosed at the bottom of the cabinetry with access panels installed with tamper proof screws. Electronically activated faucets with controlled mixing valve to be installed as well as enclosed grab bars. Replace all shower knobs with push button control with concealed mixing valve along with a timer for water amount control. Each existing mirror will be replaced with a plastic mirror, as well as removing the shelving in the restrooms. Area of consideration is restricted to firms located within a 100-mile radius of VAMC Mountain Home, TN, and IS RESTRICTED to Emerging Small Businesses in accordance with the Small Business Competitiveness Demonstration Program. NAICS Code 541310, with a small business size standard of $4.5 million is applicable. A/E selection based on recent specialized hospital experience, professional capacity to accomplish work, professional qualifications of staff and consultants assigned to the project, proposed management plan, cost control, estimating effectiveness, schedule control and location of A/E firm and consultants. This notice issued subject to availability of funds and is not a request for proposal. The estimated construction cost range is $200,000 to $250,000 with 20% of the total construction budget as viable deductive construction alternates. Interested firms should submit two copies each of current SF-330 to Attn: Sandra Magers, Contracting Officer (90C), VISN 9 Acquisition Center, Bldg. 5, Room 149, 3400 Lebanon Pike, Murfreesboro, TN 37129 not later than 4:00 pm CST on January 22, 2008 in accordance with numbered notes 24 and 28.
