Description
The VA Great Lakes Acquisition Center intends to negotiate a sole source contract with Thoratec Corporation, Pleasanton, CA to provide rental unit(s) of the PBU system (Ventricular Assist Device), associated supplies, and services on a as need basis for 1 year with two (2) one (1) year option years for VAMC MADISON, 2500 OVERLOOK TERRACE, MADISON, WI 53705-2254. The Thoratec PBU System is required for direct patient care and the patient will need to stay on this equipment until a donated heart becomes available. It is imperative that the facility have a Thoratec PBU System unit readily available before the pre-transplant heart patient arrives on station from the University of WI where the Thoratec PBU System has been implanted in the patient. In addition, to the best of our knowledge, no other equipment is compatible except the Thoratec PBU System. This procurement is being conducted in accordance FAR 6.302-1, only one responsible source and no other supplies or services will satisfy the agency requirements. Contract is firm fixed price and the NAICS Code is 334510. This Notice of Intent is not a request for competitive proposals. No solicitations documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service, to the Contracting Officer within seven (7) calendar days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information must be sent to the Contracting Officer, VAMC GLAC, 5000 W. National Ave Bldg. 5, Milwaukee WI 53295 or via fax to (414) 902-5420. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with Thoratec Corporation.
