- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subjpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Request for Quotations (RFQ) solicitation; quotes are being requested and a written solicitation will not be issued. This RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-21. This requirement is a 100% Small Business, 100% Buy Indian Set-Aside. FAR 52.212-1, Instructions to Offerors-Commercial; FAR 52.212-4, Contract Terms and Conditions-commercial Items, applies to this acquisition. Qualified vendors may submit a quote via e-mail (sara.marcus@ihs.gov) or fax 405-951-3771. Quote shall include: Updated resume with 3 references listed for the proposed worker; letter stating proposed all-inclusive hourly rate; copy of Buy-Indian Certification or Certificate of Degree of Indian Blood/Tribal Membership Identification Card and a completed copy of the provision FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. Evaluation factors: technical superiority based on resume content and reference checks. Price is secondary but will be the determining factor for equal technically qualified quotes.
