Description
This is a Sources Sought Notice only. This is not a Request for Quotes/Proposals. A solicitation is not available. The tracking number for this announcement is VA-258-08-RI-0027. The Government will use responses to this Sources Sought Notice to make appropriate acquisition decisions regarding the type of procurement to be utilized to obtain the services. The purpose of this Sources Sought Notice is to identify potential Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) sources that possess the expertise, capability, and experience to provide all labor, materials, supplies, equipment, transportation, and supervision necessary to perform on site respiratory fit testing and training in accordance with the Respiratory Protection Standards 29 CFR 1910.134 for the Southern AZ VA Health Care Systems (SAVAHCS). The Department of Veterans Affairs, SAVAHCS has a requirement for on site respiratory fit testing and training using the 3M 1860 Health Care Particulate Respirator Type N-95 or equal for approximately 343 employees per year participating in the Respiratory Protection Program. Fit testing and training must be in accordance with OSHA-approved testing procedures (Quantitative or Qualitative) and media. Services requires the contractor to conduct monthly visits to provide medical evaluation screening form to the VA employees at the time of fit testing and training; collection of the form; and, review it for completeness. Contractor will also evaluate the information provided by the employee on the medical screening form to determine if the employee is a good candidate to wear a respirator or if the employee needs further medical evaluation by VA Occupational Health Staff before being allowed to wear a respirator. The SAVAHCS Occupational Health Staff will also evaluate all medical screening forms and will make the ultimate decision about an employee's fitness to wear a respirator. The form becomes the property of the SAVAHCS. The Contractor will compare the list of participants and maintain track records of VA employees that have been trained through a Contractor/VA medical facility electronic relational database. Provide a wallet size card that certifies their successful completion, to each employee that completes the training and fit testing. The wallet size cards shall be dated and will expire 365 days later. In the event of a SARS emergency the contractor is expected to be on station to perform training and fit testing to approximately 250 more people within 24 hrs of notification. All services are will be performed at SAVAHCS, travel will not be reimbursed. All eligible SDVOSB & VOSB firms responding to this Sources Sought Notice must ensure their response is complete and sufficiently detailed to allow the Government to determine the firms' qualifications to perform this requirement. The firm must have experience providing the same services. The minimum level of acceptable experience required is 24-months within the previous 36 months. Interested firms must provide a Capability Statement with the following information: Company Name, Address, Point of Contact, Telephone and Fax Number, Email Address, Business Size, and DUNS Number. Past Performance: List your three most recent contract awards for like or similar work, to include Client's Name, Point of Contact, Phone Number, Description of Services, Dollar Amount, and Period of Performance. This market research does not constitute an invitation for bid or request for proposal and is not to be construed as a commitment by the Government. The Government will not reimburse firms for any cost incurred for their participation as an interested party. All responses must be submitted by email to Lillian.Provencio@va.gov; no other method of transmittal will be accepted. All capability statements and questions must be received by Dec 12, 2007. Telephonic requests will not be acceptable. The North American Industry Classification System (NAICS) code is 561720, and the Small Business Size Standard is $15M.
