See Numbered Note 22 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This is solicitation number W91YTZ-08-R-0022 and is a request for quo tation (RFQ). This is a total Small Business Set-Aside. The Standard Industrial Classification Code is 5995 and the small business size standard is $6.5M. The North American Industry Classification System Code is 446130. USA MEDDAC, Fort Knox, KY has a req uirement for two (2) Prescription Eyeglass Makers. This will be a base with four 1 year option periods. There will be 4 CLINS with a qty of 3,840 hours per year (1,920 for each person). All offers received in response to this solicitation will be evaluate d. Responses are to be emailed to joyce.ambrose@us.army.mil no later than 0900 hours 18 December 2007. Please be advised that all contractors doing business with the Department of Defense must be registered with CCR (the website may be accessed on the inte rnet at: www.ccr.gov to register). Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Provision 52.212-1, Instructions to Offeror-Commercial Items. As stated in this provision, the G overnment intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial offer should contain the offerors best terms from a cost or price standpoint. However, the Government reserves the right to conduct discussions if later and determined by the Contracting Officer to be necessary.
