Description
This is not a solicitation announcement and this notice does not constitute any commitment by the Government. THE PURPOSE OF THIS MARKET RESEARCH AND SOURCES SOUGHT NOTICE IS TO GAIN KNOWLEDGE OF POTENTIAL QUALIFIED SMALL BUSINESS OWNED FIRMS. ONLY SMALL BUSINESS FIRMS SHALL RESPOND TO THIS NOTICE. Responses to this Sources Sought Synopsis will be used by the government to make appropriate acquisition decisions. The U.S. Army Corps of Engineers, Mobile District, anticipates a future procurement rel ated to the FY-08 Regional Physician Services for Multiple State Physician Contract to promote and maintain the physical and mental fitness of U.S. Army Corps of Engineers employees and outside federal agency customers. The Scope of Work will consist of t hree (3) main tasks. (1). Physical Examinations. Occupational Health Physician (OHP-medical doctor) shall conduct a hands-on physical examination for employees who require mandated medical surveillance exams, or voluntary physicals. The physical examin ations will be conducted in the US Army Corps of Engineers clinic in Jacksonville, Savannah or Mobile District. The Physician shall provide on-site services for up to four hours per week. Medical Protocol, the OHP will confirm and approve Medical protocol s for the District Occupational Health Nurses (OHN) who will staff the Unit when the Physician is /is not present. Medical Surveillance. Under this tasking, the OHP shall review physicals performed by other physicians when the District OHN deems addition al attention is warranted. The OHP will provide medical advice to Industrial Hygienists as needed for exposures or potential exposures to employees; and review specific medical surveillance examinations based on work related exposures. Provide consultati on of Safety on Medical/Safety issues both in person and via telephone. (2). Physician Services (Occupational Health Physician preferred) are needed PRN for oversight and assistance with disaster response missions and other health promotions provided by USACE registered nurses for Federal Employees for CONUS and OCONUS locati ons but primarily in the following states: Alabama, Florida, Georgia, Mississippi, North Carolina, South Carolina, Tennessee, Texas, Puerto Rico, Virgin Islands, Louisiana, and Virginia. (3). Physician services are needed PRN for AED oversight both CONUS and OCONUS but primarily in the following states: Alabama, Florida, Georgia, Mississippi, Tennessee, North Carolina, South Carolina, Puerto Rico and the Virgin Islands. The size standard for which is a concern, including its affiliates whose average annu al gross revenue does not exceed $9.0 million for the past three (3) years under a firms with a North American Industry Classification System (NAICS) Code of 621111, Offices of Physicians (except Mental Health Specialists). The Government contemplates aw arding a Firm- Fixed-Price IDIQ Service Contract for a future solicitation. Interested firms should submit document describing relevant experience in the following format: name(s) of client(s); location of job site; client point of contact and telephone number; type of project, estimated value of project, and the date projects were performed. This documentation should be limited to descriptions of past and current contracts of the kind of work and facility type identified above and should be limited to e ight (8) pages. It is not necessary to submit additional company information, resumes, sales material or brochures. Responses should be submitted via email to Mrs. Shirley Y.
