Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W7795173200001 is issued as a Request for Quote (RFQ). This request for Quote is due by 27 November, 12:00pm (local time). The RFQ will result in a firm fixed price contract. This solicitation is 100% set aside for small business. The North American Industry Classification System (NAICS) code for this acquisition is 621210. The United States Property and Fiscal Office of the Washington Army National Guard is solicit ing quotes to procure the following supply or services: CLIN 0001: Dental Exams Seattle armory for 125 personnel. QUANTITY 125EA. PRICE____________. CLIN 0002: Dental Exams Yakima armory for 65 personnel. QUANTITY 65EA. PRICE____________. STATEMENT OF WORK FOR SEATTLE 1. OSHA compliant mobile dental examination services in the Seattle Armory 1601 West Armory Way Seattle WA, 98119. Anticipated number of exams is 125 per day. There will be space inside the armory building to set up dental equipment so a dental van is n ot necessarily required. Use of a van or not should be determined by best cost. 2. Services to include annual dental exam per AR 40-501 with bitewing films. Panograph x-rays will also be taken if the soldier has none in their military dental record or the x-ray on file is no longer current. Generally, panograph x-rays are considere d current provided there have been no significant changes in the mouth since the last film. 3. Work day will be a maximum of 10 hours/day. Anticipate exams to begin at 0800. 4. If the soldier's current dental exam is 6-9 months old or older, a new dental exam will be done. 5. A copy of the sign-in log of all soldiers will be delivered in person, by fax or by mail to the Deputy State Surgeon. The sign in log must include the following information: Soldier's name, SSN Dental Classification Is there a current pano film on record? Yes/no (If a pano film is taken that day, mark yes) 7. Denclass input is requested. Therefore need a contractor with Denclass capacity. 8. POC is the Deput State Surgeon, LTC Carol Munsey at 253-512-8993 or carol.munsey@wa.ngb.army.mil.
