- Description
- The Air Force Institute for Operational Health, Epidemiological Surveillance Division (AFIOH/SDE) intends to negotiate with Gen-Probe Inc., 10210 Genetic Center Dr, San Diego, CA 92121 on a sole source basis for Chlamydia trachomatis and Neisseria gonorroheae reagent testing kits using Nucleic Acid Amplification technology. Estimated Quantities : 26 Aptima Combo 2 Assay kits, 2 CT verification kits, and 2 GC verification kits. Required delivery is FOB: Destination. AFIOH/SDE, 2513 Kennedy Circle, Brooks City-Base, TX 78235 within 14 days after contract award. Authority for soliciting sole source is FAR 6.302-1, Only one responsible source and no other supplies or services will satisfy requirements, because Gen-Probe is the only one that can provide the required test kits that can be used on Gen-Probe Tigris DTS system. This is a combined synopsis/solicitation prepared in accordance with the format in Far Subpart 12.6, as supplemented with additional information included in this notice. The provision at FAR 52.212-1 Instructions to Offerors-Commercial applies to this acquisition. Offerors are to include a completed copy of FAR provision 52.212-3 Offeror Representations and Certifications - Commercial Items with their response. The following FAR clauses also apply: 52.212-4 -Contract Terms and Conditions - Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, with the following clauses cited in 52.212-5 being applicable: 52.222-3 Convict Labor, 52.222-21 Prohibition of Segregated Facilities, 52.222-26 Equal Opportunity, 52.222-35 Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans, 52.222-36 Affirmative Action for Workers with Disabilities, 52.222-37 Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans, 52.225-13 Restrictions on Certain Foreign Purchases, and 52.232-33 Payment of Electronic Funds Transfer - Central Contractor Registration.
