- The following Veteran Administration Medical Outpatient Clinic (Knox OPC) located at 9031 Cross Park Drive, Knoxville, TN 37923 will be providing a solicitation for Medical Waste Disposal Services, NAICS code 562112. Services to be provided shall include plastic containers for bio-hazardous infectious waste, all supplies including red bags for bio- hazardous infectious waste, and labels for bio- hazardous infectious waste. The contractor shall provide pick-up once a week, day and time to be determined by the COTR after contract award. The contractor will keep a log/stop sheet provided by the VA certifying completion of each pick-up. All waste picked -up will be bar coded to facilitate tracking until incinerated/autoclaved or transferred to a licensed company that will be responsible for the bio- hazardous infectious waste disposal. Treatment of bio-hazardous infectious waste must be in accordance with the State, Federal and EPA solid and hazardous waste laws and regulations. All supplies and equipment shall comply with the applicable federal, state, county, and municipal laws, regulations, guidelines and permits Contractor shall provide Medical Waste Disposal Services for the following periods: Base Year period (9 months) January 1, 2008 through September 30, 2008 Option Year #1 October 01, 2008 -September 30, 2009 Option Year #2 October 01, 2009 - September 30, 2010). This Presolicitation Notice is being posted in anticipation of RFP VA-249-07-RQ-0165. The solicitation shall be posted on or about November 16, 2007 with responses due December 07, 2007.
