Submit your technical and cost proposal to accomplish this Task Request to the Contracting Officer by 12:00pm, November 24, 2007. Specifications and requirements are attached. Your cost proposal must be on a fixed price basis using the rates as contained in the referenced contract. This price will include all associated costs relating to this task including background security investigations, travel and perdiem costs. Submitted by: _______________________________________ NAME ________________________________________ SIGNATURE DATE STATEMENT OF WORK FOR Cross Sectional/General Diagnostic Radiologist Services DESCRIPTION OF SERVICES: 2.0 Statement of Work A. Definition of FTE: a 1.0 full time equivalent (FTE) is defined as a contractor's personnel (service provider) working eight (8) hours per day, five (5) days per week. A contractor's personnel (service provider) must be present at the VA facility and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A record keeping system of contractor hours worked will be established. The payments for any leave, including sick leave, holiday or vacation time, are the responsibility of the Contractor. B. Contractor shall provide a board certified licensed General Diagnostic Radiologist, BCLS Certified to perform services which include, but are not limited to: 1. Interpreting plain film, ultrasound, CT, MRI and nuclear medicine, and be proficient in fluoroscopy. 2. Dictate reports into VA Central Dictation System and sign transcribed reports within 48 hours. 3. Perform Computerized Medical Record entry. The White River Jct., VA maintains a computerized medical records system, individuals performing work under this agreement must be computer literate and be proficient in keyboard and word processing skills. 2.1 Special Contract Requirements A. The services to be performed by the contractor will be performed within the VA policies and procedures and the regulations of the medical staff by-laws of the VA facility. Work performed under this contract will be evaluated in accordance with facility competency assessment worksheet on the following performance factors: Clinical Knowledge, Clinical Competence/Skills, Emotional Stability, Interpersonal Relationship, Knowledge of Client Age Specific Needs, Administrative Citizenship. In the event that a service provider is rated at an unsatisfactory level the VA may request removal and replacement of such provider. B. The services to be performed by contractor personnel will be under the direction of the Chief, Radiological Service, VA Medical Center, White River Jct., VT. Individuals who have been approved to provide services under this contract will be responsible to report absences (due to illness, etc.) to the Chief, Radiological Service. Contractor is required to provide the required coverage in the event absences of 24 hours or more are anticipated. Credentials of any replacement service provider must be forwarded to the Contracting Officer for approval prior to services being authorized under this contract. C. All personnel providing services under this contract shall provide evidence of current TB testing, and have received and training in infection control procedures. Evidence of such testing and training shall be submitted prior to commencing work at the facility. D. Other necessary personnel for the operation of the services contracted for at the VA will be provided by the VA at levels mutually agreed upon which are compatible with the safety of the patient and personnel and with quality medical care programming. E. Personnel assigned by the contractor to perform the services covered by this contract shall be Board Certified with current BCLS. The qualifications of such personnel shall also be subject to review by the VA Medical Director and approval by the VA Center Director. Contractor will be required to have all providers furnishing services under this contract prepare all forms required by third party payers so that the VA can bill for the professional services rendered by the contractor's providers. F. Clinical or other medical records of VA beneficiaries treated by the contractor will remain the property of the VA. G. Key Personnel And Temporary Emergency Substitutions: (1) During the first 30 days of performance the Contractor shall make NO substitutions of key personnel unless the substitution is necessitated by illness, death, or termination of employment. The Contractor shall notify the Contracting Officer, in writing, within 15 calendar days after the occurrence of any of these events and provide the information required by paragraph (3) below. After the initial 30-day period of the contract, the Contractor shall submit the information required by paragraph (3) to the Contracting Officer at least 15 days prior to making any permanent substitutions. (2) The Contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete resumes for the proposed substitutes, and any additional information requested by the Contracting Officer. Proposed substitutes shall have comparable qualifications to those of the persons being replaced. The Contracting Officer will notify the Contractor within 15 calendar days after receipt of all required information of the decision on the proposed substitutes. The contract will be modified to reflect any approved changes of key personnel. (3) For temporary substitutions where key person will not be reporting to work for three (3) consecutive work days or more, the Contractor will provide a qualified replacement for the key person. This substitute shall have comparable qualifications to the key person. Any period exceeding two weeks will require the procedure stated in paragraph (b) above. H .All work must be performed in accordance with JCAHO agency standards. A copy of these standards may be obtained from the Joint Commission on Accreditation of Healthcare Organizations, One Renaissance Blvd., Oakbrook Terrace, IL 60181. (1)The Contractor will be responsible to ensure that Contractor personnel (service provider) providing work on this contract are full trained and completely competent to perform the required work. All certifications need to current through the term of the contract and if renewal is required it will be the contractor's responsibility to insure it is accomplished without interruption to services being provided. (2)The Contractor is required to maintain records that document competence/performance level of Contractor personnel (service provider) working on this contract in accordance with JCAHO and other regulatory body requirements. The Contractor will provide a current copy of the competence assessment checklist and annual performance evaluation to the Contracting Officer's Technical Representative (COTR) for each Contractor personnel (service provider) working on this contract. (3)When changes in Contractor personnel are approved in accordance with the "Key Personnel" clause of the contract, the Contractors must provide the providers name, a current address and telephone number, Social Security Number, list of references, list of all training, evidence of an unrestricted license, the current competence assessment, and current performance evaluation that suppers #1 above. (4)The Contractor is required to develop and maintain the following documents for each Contractor personnel (service provider) working on the contract: credentials and qualifications for the job; a current competence assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform a job correctly and skillfully; includes knowledge and skills required to provide care for certain patient populations, as appropriate.); a current performance evaluation supporting ability of the Contractor personnel (service provider) to successfully perform the work required in this solicitation; and listing of relevant continuing education for the last two years. The Contractor will provide current copies of these records at the time of contract award and annually on the anniversary date of contract award to the VA COTR, or upon request, for each Contractor personnel (service provider) working on the contract. 3.0 Contract Administration 3.1. Term of Contract This contract is effective from December 16, 2007. This contract may be amended at the option of the VA. The total length of this contract shall not exceed the last option dated July 20, 2008. This contract is subject to the availability of VA funds. No service shall be performed by the contractor July 20, 2008 until the contracting officer authorizes such services in writing. If the number of FTE provided under this contract is reduced, the contract price will be reduced in direct ratio to the reduction in FTE. 3.2 Qualifications Personnel assigned by the contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to review by the VA Medical Director and approval by the VA Facility Director. 3.3 Work Hours Work Hours: The services covered by this contract shall be furnished by the contractor as defined herein. The contractor will not be required, except in case of emergency, to furnish such services on a national holiday or during off duty hours as described below which the following meanings have: a. Work Hours - Monday through Friday 08:00am to 4:30pm b. National Holidays: The ten (10) holidays observed by the Federal Government are as follows: New Year's Day Martin Luther King's Birthday Washington's Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas Any other day specifically declared by the President of the United States to be a national holiday. c. Off Duty Hours: Monday through Thursday, 4:30pm - 8:00am, Friday 4:30pm - 8am, and all Saturday and Sundays. 3.4 Personnel Policy The contractor shall be responsible for protecting the personnel furnishing services under this contract. Thus, the contractor shall be responsible for ensuring that the contractor and/or the individual service provider complies with all laws and regulations applicable to labor and employment including but not limited to the following: Worker's compensation; Professional liability insurance; Health examinations; income tax withholding; Social security payments The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered personnel (service provider) of the contractor. Medical Liability Insurance will be at the rates identified in the Federal Supply Schedule. 3.5 Payment A. The VA agrees to reimburse the Contractor on a unit basis for resources furnished at the prices listed in the Schedule of Supplies/Services portion of this contract. B. Payment of sums due will be paid monthly upon submission (by the contractor) of a properly prepared invoice and receipt in the appropriate VA office. The VA will review the records for agreement with the VA records and verify that the coverage provided for the month is in accordance with the contract. The VA upon verification by the VA of the services billed will authorize payment. C. Billings rendered by the contractor to the VA for services furnished a VA beneficiary under the terms of this contract shall be billings in full. Neither the beneficiary, his/her insurer nor any other third party shall be billed. D. In the event that coverage is not provided in accordance with this contract, the payment to the contractor shall be reduced in direct ratio to the reduction in coverage. Any additional costs incurred by the VA as a result of obtaining coverage from a source other than the contractor, due to the contractor's failure to provide adequate coverage, will be assessed against the contractor. 3.6 Recordkeeping A recordkeeping system, by use of time and attendance logs, will be established at this VA to demonstrate that the VA has received services called for under the contract. The Medical Service Secretary will maintain these logs. Contractor's service provider shall sign in and out with the Medical Service Secretary when coming on duty and when leaving duty. In the event that work is performed during off duty hours, sign in will be accomplished with the Administrative Officer of the Day (AOD). 3.7 Termination The Government may terminate this contract at any time upon at least 15 days' written notice by the Contracting Officer to the contractor. The contractor, with the written consent of the Contracting Officer, may terminate this contract upon at least 30 days' written notice to the Contracting Officer. 3.8. Contractor Personnel Security Requirements All contractor personnel (service provider) who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for VA. a. Position Sensitivity - The position sensitivity has been designated as Low Risk. b. Background Investigation - the level of background investigation commensurate with the required level of access is Minimum Background Investigation. c. Contractor Responsibilities 1) The contractor shall bear the expense of obtaining background investigations. If the Office of Personnel Management (OPM) conducts the investigation, the contractor shall reimburse VA within 90 days. 2) The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are able to read, write, speak and understand the English language. 3) The contractor shall submit or have their personnel (service provider) submit the following required forms to the VA Office of Security and Law Enforcement within 30 days of receipt: (i) Standard Form 85P, Questionnaire for Public Trust Positions (ii) Standard Form 85P-S, Supplemental Questionnaire for Selected Positions. (iii) FD 258, U.S. Department of Justice Fingerprint Applicant Chart (iv) VA Form 0710, authority for Release of Information form (v) Optional Form 305, Declaration for Federal Employment (vi) Optional Form 512, Optional Application for Federal Employment 4) Personnel (service provider) working under this contract will also be required to furnish the following prior to or at arrival at this facility. (i) A State-issued driver's license or State-issued identification containing a photograph. If the driver's license or identification does not contain a photograph, identifying information should be included such as: name, date of birth, sex, height, color of eyes, and address, or a Military ID Card. (ii) A social security card other than the one which has printed on its face "not valid for employment purposes", OR an original or certified copy of a birth certificate issued by a state, county or municipal authority bearing a seal. (iii) If they have one of the documents listed below, this is the only information they are required to bring. Any of these documents will establish their identity. a) U. S. Passport b) Certificate of Naturalization c) Certificate of U. S. Citizenship d) Unexpired foreign passport with attached Employment Authorization e) Alien Registration Card with photograph 5) The contractor, when notified of an unfavorable determination by the Government, shall withdraw the personnel (service provider) from consideration from working the contract. 6) Failure to comply with the contractor personnel security requirements may result in termination of the contract for default.
