- SOLICITATION NOTICE VA-69D-08-RQ-0032 MOBILE PET/CT SCAN SERVICES The GLAC (Great Lakes Acquisition Center) is soliciting quotations for Mobile PET/CT Scan Services for beneficiaries of the Department of Veterans Affairs Medical Centers (VAMC) at Edward Hines VA Hospital, Fifth Avenue & Roosevelt Road, Hines, Illinois for the period of December 1, 2007 through November 30, 2008, with up to three one-year options to extend, in accordance with all specifications, work statement, terms and conditions contained therein. All qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their proposal. Offers shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. This is a Firm Fixed Price contract and this acquisition is a 100% HUBZone Set-aside. The size standard for North American Industry Classification System (NAICS) code 621512 is $12.5M. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs.
