- The Contractor shall provide qualified physicians to provide Urology Services on site in accordance with the terms and conditions of the solicitation. This is not a locum tenens requirement. The Contractor shall provide 0.5 FTE for the VAMC, Louisville, 800 Zorn Avenue, Louisville, KY 40206. The solicitation will be available for downloading o/a October 25, 2007. All communication including questions shall be in writing will be emailed to john.gregorio@va.gov or faxed to 615-867-5429. Email is preferred. No telephone calls will be accepted or returned. This solicitation is a Total Small Business Set Aside. NAICS 621111 $9.0 million. This is commercial items procurement. The following clauses and provisions and all addendum apply: 52.212-1 Instructions to Offers, 52.212-2 Evaluations, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes or Executive Orders. The Contractor is responsible to download all amendments and other documents without further notice from the DVA. No web based offers are accepted.
