Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation HSFEMW-08-Q-0001, issued as a Request for Quotation (RFQ), is for the purchase of trash removal services, performed at FEMA's Mt. Weather Emergency Operations Center (MWEOC), 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135. The solicitation document and incorporated provisions and clauses are those in effect thru Federal Acquisition Regulation Circular (FAC NO.) 2005-20 dated 09/06/2007. The NAICS code is 562111. It is anticipated that only one award will result from this solicitation via the issuance of a firm fixed price simplified acquisition/commercial services Blanket Purchase Order to include a base year and four option year periods. The Wage Determination associated with this requirement is WD 05-2249, for Clarke County. Services are to be performed at the Federal Emergency Management Agency, Mount Weather Emergency Operations Center, 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135-2006. Evaluation Information: The Government will make an award to the responsible/responsive offeror whose offer will be most advantageous based on lowest price/technically acceptable quote. In order to be found technically acceptable, the offeror must be capable of providing the following information in order for the Government to make a determination of technical acceptability A. The Contractor shall provide a statement that it has not been terminated for default in the past three years. B. The Contractor shall provide three references in support of its capability to delivery quality work in a timely manner. The Contractor's offer should contain individual unit prices for each trash pick up and compactor container return to the site. Usually pick up and container return will be once a week. However, some weeks may require more than one pick up. All trash will consist of wood and metal. There will be a site visit at 2:00 p.m. on Tuesday, October 30, 2007. All participants must contact the Contract Specialist NLT Monday October 29, 2007 by 12:00 PMEST with an email address or fax number where a required security form can be sent for admission to this secured site. The provision at FAR 52.212-1 Instructions to Offerors-Commercial Items, applies to this acquisition.
