- This requirement is cancelled in it entirety as of 10/12/2007. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. TFMSHQ08Q0025 is being issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-23. The National American Industry Classification System code is 51331 with a corresponding size standard of $6.0. The U.S. Department of the Treasury, Financial Management Service (FMS), 401 14th Street, SW, Washington, DC, 20227, intends to award a purchase order utilizing simplified acquisition procedures and provisions and clauses for Commercial Items to Bellsouth Telecommunications, Inc., who will provide monthly telephone service to the Financial Management Service, Birmingham Debt Management Operations Center, 190 Vulcan Road, Homewood, AL, 35209. The requirement is for Voice Telephone Service. Coverage shall be for twenty-four (24) hours per day, seven (7) days per week, 365 days per year. The approximate number of lines is (300). FMS will reserve the right to add or delete voice telephone service for the life of the order. The government will not pay for installation of new lines. FAR 52.212-1, Instructions to Offerors-Commercial applies t this acquisition; FAR 52.212-2, Evaluation-Commercial Items is hereby incorporated by reference. The government will award a commercial item purchase order using simplified acquisition procedures to the responsible offeror whose offer is most advantageous to the government. The offers will be evaluated on the basis of system requirement supportability, past performance and price. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items applies to this acquisition. FAR 52.21-4 Contract Terms and Conditions-Commercial Items. Addenda to 52.212-4 include: FAR 52.217-9, Option to Extend the Terms of the Contract (Mar 1989); -FAR 52.222-37, VETS 100 Compliance Report. The clauses may be viewed at http://www.arnet.gov/far. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial items applies to this solicitation. Past performance surveys can be obtained from Evelyn J. Daval, Contract Specialist. Offers are to be received no later than 10am, October 19, 2007. The period of performance is from Date of Award through September 30, 2008 with four (4) 1-year options: Option Year 1, October 1, 2008 through September 30, 2095 and Option Year 2, October 1, 2009 through September 30, 2010, Option Year 3, October 1, 2010 through September 30, 2011, Option Year 4, October 1, 2011 through September 30, 2012. Response must be in writing by above-mentioned date. Delivery should be with in 30 days after Receipt of Order, (A.R.O). Responses must be in writing. Responses must include the Representations and Certifications and the VETS 100 Compliance Report.
