- ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 12 Months Healthstream annual contract FFP for current system. See Section C for Statement of Work. FOB: Destination MILSTRIP: N0018308RQT5001 PURCHASE REQUEST NUMBER: N0018308RQT5001 NET AMT Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT THE MINIMUM REQUIREMENTS OF THIS SOLICITATION ARE AS FOLLOWS: ? Vendor to provide ?Healthcare Learning CenterTM?, manufactured by Healthstream Corp. Supplied software / system must be able to support a minimum 5,000 students. o Currently Naval Medical Center Portsmouth (NCMP) is using the following products manufactured by Healthstream INC: Authoring Extension Kit (CP-2001-A) & Healthcare Education System Network (CCP-2009N 5000-5999). Vendor will be required to transfer any and all required files to the newly installed system. Additionally vendor must ensure that there is no unscheduled down time during switch over between the new and existing system. ?X Down times during change over must be kept to a minimum and scheduled after normal work hours of 0730 to 1600 Monday through Friday. (unless otherwise approved by project coordinator / end user) ?X All transferred data must be protected to ensure that no release is made to an unauthorized agent. ?X Upon successful testing and implementation of the ?Healthcare Learning CenterTM? system selector vendor must provide disconnection of existing system and turn over any required hardware items to NMCP for release back to Healthstream. o Vendors price to inclusive of all cost including; software license, labor and shipping. o Vendor is to warrant that all supplied items are complete and acceptable for use. All supplied software must be ?virus free? and compatible for use with NMCP computer systems. ?X All web based access points must be supplied with a means to prevent unauthorized access. IE: ?Gatekeeper? installed to prevent hacking. ?X Selected vendor to ensure that any and all collected data to be used for the ?good of the Government only?. No release of data allowed. ?X Naturalized citizens of the United States operating within the United States must conduct all technical support operations which involve accessing of Government computer systems. Selected vendor is allowed to request a wavier of this requirement, but must obtain written permission from the resident Contracting Officer. ? All supplied software and hardware must be of the most current version as of the date of contract award. ? Information / data supplied must be certified for use in US adhering to UL and JACHO requirements as required. Contact Harold Woodley for terms and conditions.
