- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Proposal (RFP) No.FA7014-08-T-0001. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-05. This is a small business set-aside. The North American Industry Classification System Code is 611430 at $6.5 Million a year earnings size standard. The period of performance will be a base period starting November 6-9 2007. PERIOD OF PERFORMANCE: CLIN 0001 Full Mock Survey QTY 1, UNIT Each UNIT PRICE _______ IAW Statement of Work. (See Instructions to Offerers, Description from the Statement of Work, and Bid Schedule). Also, the contractor shall be responsible for any FAR, DFARS AFFARS PROVISIONS: The provisions at 52.212-1, Instructions to Offerors; 52.212-2 Evaluation -- Commercial Items; 52.212-3, Offerors Representation and Certifications; 52.219-6, Notice of Total Small Business Set-aside; 52.242-15, Stop-Work Order; 52.247-34, F.O.B Destination; 252.204-7004, Required Central Contractor Registration-Commercial Items are hereby incorporated. Addendum to FAR Clause 52.212-1 applies to this acquisition as follows: 52.252-1. Offeror must include, with its offer, a completed copy of provision FAR 52.212-3 in full text. FAR CLAUSES: The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, is hereby incorporated. FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Status of Executive Orders-Commercial Items apply to this acquisition. It is greatly preferred that questions be submitted by email rather than phone. DESCRIPTION OF SERVICES The 79th Medical Group requires the services of an external source to perform an on-site full mock survey, life safety code follow-up survey and staff education. The mock survey shall be performed by a skilled contractor-clinician who possesses deep knowledge of Air Force Military Treatment Facilities and accreditation standards for hospitals. Ti is requested that the external source possess experience in the techniques employed by The Joint commission in conducting actual Joint Commission Surveys. At the conclusion of the engagement, a mock survey report will be provided to the Commander, 79th Medical Group. This report will comprehensively address areas identified by the mock survey, and accurately represent the state of preparedness of the 79th Medical Group. The services of the external source are requested from 6 November 2007 though 9 November 2007.
