Bldg 723, 8150 Aeromedical Drive, San Antonio TX 78235. 311 MSG/LGBE Medical Logistics Squadron at Brooks City-Base in San Antonio, TX This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a requirements package is attached. The reference number for this acquisition is FA8901-07-R-0004. The North American Industry Classification System (NAICS) code is 334519 and the (SB) size standard is 500. The following clauses and provisions are incorporated by reference and are to remain in full force in any resultant contract or as deemed necessary by the cognizant contracting officer; FAR 52.212-1, Instructions to Offerors, Commercial Item (Sep 2006) applies to this acquisition. Offerors must be registered in the Online Representations and Certifications system at http://orca.bpn.gov and provide a completed DFARS 252.225-7001, Buy American Act-Balance of Payment Program Certificate (Jun 2005) with the quote as applicable. Clauses and provisions may be accessed via the Internet, http://farsite.hill.af.mil. The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items (Sep 2005) is incorporated into this RFP and is amended to add the following clauses: FAR 52.204-4, Printed or Copied Double-Sided on Recycled Paper (Aug 2000) and FAR 52.204-7 Central Contractor Registration (Jul 2006) The solicitation number, FA8901-07-M-0172, will be made available on the Government Point of Entry (GPE), Federal Business Opportunities (FBO) web site at http://www.fbo.gov. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). All responsible business offerors will be considered by the agency. This commercial acquisition is for purchase of Two Each Reduced Oxygen Breathing Device 2 (ROBD2), double stage medical grade regulators for medical grade gases- 3 per system, and braided high pressure hose 10 feet long - 3 per system, color-coded with quick disconnects, and heavy duty sipping case with wheels. Additional components required include the following: 1) Pulse Oximetry Finger Sensor 2) Pulse Oximetry Y (ear) Sensor 3) 02 Sensor Repalcement with article filter/fixed orifice 4) 10 Ear slips for Pulse Oximeter Sensor. The type of contract anticipated is firm-fixed price.
