- 1) Mouth Guard, Adult, Gel Max, Exoskeletal Shock Frame, Gel Fit Liner, Breathing Channel, Black, Shock Doctor Part No. 6100A, or equal - 4,998 each; 2) Mouth Guard Case, Heavy Duty, Antibacterial, Ventilated, Shock Doctor Part No. 101C, or equal - 4,998 each. The dental warranty offered must be equal to the Shock Doctor warranty. Freight/shipping charges shall be specified on the quote as a separate line item. Please provide detailed information on items quoted. This is a combined synopsis/request for quotation for commercial items. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. FAR Clauses 52.212-4 and 52.212-5 are applicable to this acquisition. The following clauses are checked: 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-1, 52.225-13 and 52.232-33. Offerors are reminded to include a completed copy of the Provisions at FAR 52.212-3 Offeror Representations and Certifications-Commercial Items, with its offer.
