- RFP VA-528-07-RP-0207 is issued to provide primary care services at a location in Jefferson County in the State of New York. The Veterans Affairs Healthcare Network Upstate New York (VAHNUNY) desires to enter into a contract for community based primary/preventive care services for veterans. The resulting contract will be for a one-year period with two one-year renewal options to commence on or about November 1, 2007. The Contractor will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology and administrative functions to fulfill the level of care specified in the RFP. Service will be at an all-inclusive per visit rate for an estimated 8400 visits per year. The NAIC code is 62111 and the Commercial items solicitation is unrestricted.
