Install sheet rock at end of lockers. ***SEE COMPLETE STATEMENT OF WORK IN SECTION C*** FOB: Destination MILSTRIP: N6275307RQD0050 PURCHASE REQUEST NUMBER: N6275307RQD0050 NET AMT Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT Statement of Work 1. BACKGROUND Naval Branch Dental Clinic Norfolk has over 260 personnel onboard that support Naval Station Norfolk and surrounding areas. In order to secure personal belongs during treatment hours, adequate locker space is required for each individual. The current locker rooms are equipped with deteriorated and broken lockers of different dimensions which do not allow for proper storage of clothing articles and personal effects. Naval Branch Dental Clinic Norfolk requests remodeling of all locker spaces. 2. OBJECTIVES The objective of this Statement of Work is to assure all equipment, liability; maintenance, storage, security, and movement of old and new lockers are fully explained. 3. CONTRACTOR RESPONSIBILITIES 3.1. The contractor shall provide all necessary personnel to remove, deliver, and set up, requested items as outlined in sections 4. 3.2. Contractor?s Representative. The contractor shall provide the name, phone number and email address of the Contractor?s representative upon award. The contractor?s representative shall be available during the working hours to discuss with the Naval Branch Dental Clinic Norfolk, any issues that may arise concerning contract matters relating to daily operations of the contract. Working hours are requested to be after hours or on the weekends to accommodate staff members and avoid interruptions as needed with storage. 3.3. The Contractor?s representative shall be responsible for: 3.3.1 Communicating with the Naval Branch Dental Clinic Norfolk on all aspects relating to contract. 3.3.2. Resolving technical issues with the removal and or installation of lockers. 3.3.3. Responding to Naval Branch Dental Clinic Norfolk, concerns during the removal and installation period. 3.4 Notify and obtain approval from the Facilities Department at Naval Medical Center Portsmouth before making any structural changes to locker room 4. SPECIFIC TASKS 4.1. Install approximately 353 metal lockers in 7 locker rooms within 3 weeks after contract commencement. This 3 week period is to include removal of old lockers as well. 4.2 Remove and dispose of existing lockers in 7 locker rooms. 4.3 Install sheet rock at end of lockers, paint color to match lockers, if needed. Any work involving change to building structure must first be cleared through the Facilities Department at NMCP Portsmouth. 4.4. Lockers are to be 3 wide, 72"Hx36"Wx18"D, sand color. 4.5. If needed install individual lockers, 72"Hx12"Wx18"D, sand color. 4.6. Setup of contractors? equipment is the sole responsibility of the contractor. 4.7. Breakdown of the contractors? equipment is the sole responsibility of the contractor. 4.8. A list of all needed maintenance will be provided to assure equipment maintains full production capabilities at all times. 4.9. Contractor must contact the contracting department at Naval Medical Center Portsmouth if any issues arise with installation 4.6. Contractor shall be licensed on any services rendered to equipment during contract period. 5. OTHER CONDITIONS/ SECURITY 5.1. Safety: The contractor has established a safety and health program in accordance with the standards of the Federal Occupational Safety and Health Standard (OSHA), Health Information Portability and Accountability Act (HIPPA), and The Privacy Act. 5.2. Security Requirements: Base security will require all employees and affiliated personnel of the contractor to carry a State recognized identification card or drivers license and all vehicles are subject to search without reason. The contractor will comply with all installation security requirements.?(Base access for Property Manager or Facilities?) 5.3. Local property manager will be available upon 24 hour notice for facilities issues. The contractor will be held accountable for any missing or damaged items.
