Description
Description This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, in conjunction with FAR 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; and 52.212-4 Contract Terms and Conditions - Commercial Items. NAICS Code for this requirement is 339113. The Louis Stokes Cleveland VAMC, Brecksville Division, Cleveland, Ohio 44140 intends to negotiate a sole-source contract with Abbott Vascular, Inc. for Stents and Balloon Catheters. However, any firm that believes it can meet the requirements may give written notification to the Point of Contact (POC). Supporting documents must be furnished in sufficient detail, to demonstrate the contractor's capability to provide and ability to comply with the requirements. Contractor must be registered in Central Contractor Registration. [Note: Lack of registration in the Central Contractor Registration will make a Contractor ineligible for award] Registration is available via the Internet at www.ccr.gov. Shipping shall be direct to Chief AMMS 90C (B), 10701 East Blvd, Cleveland, OH 44141. The requirements includes: Fox SV (small vessel) PTA Catheter (0.018)(international), 2.5mm x 40mm, 2.5mm x 60mm, 4mm x 30mm, 4mm x 60mm, 4mm x 80mm, 5mm x 60mm; Veripath Peripheral Guiding Cather (international); Exceed Self Expanding Stent System (0.035) (international) in 5mm x 30mm-2 ea and 5 mm x 60mm-2 ea; Xpert Self Expanding Stent System (0.018) (international)in 4mm x 20mm-ea, 4mm x 40mm-2ea, 4mm x 60mm-2 ea;. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Price. Quotes must be submitted to the POC no later than Sept 7, 2007; 12 pm; EST. Quotes shall be submitted via e-mail in Word, to the POC, Jean Mays, at Jeanette.Mays@va.gov or faxed to 440-838-6052. Responses received will be evaluated however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, LS Cleveland VA Medical Center will proceed with the sole source procurement.
