Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format listed in FAR Subpart 12.6, and as supplemented with additional information included in this notice. This announcement constitutes the only solicita tion; bids are being requested and a written solicitation will not be issued. The solicitation number is W45C0672180109 and is issued as a Request for Proposal (RFP), unless otherwise indicated herein. The solicitation document and incorporated provision s and clauses are those in effect through Federal Acquisition Circular. The associated North American Industrial Classification System (NAICS) code for this procurement is 621910 with no size standard specified. The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on September 21, 2007 16:00:00.0 Eastern Standard Time (EST) or as otherwise displayed at www.FedBid.com. The Fort Bliss Contracting Office requires the following services: Contractor shall provide all personnel, uniforms, administration support, and supervision required to manage and operate the 24/7 Emergency Ambulance Services for two (2) ambulances at McGregor Range, Dona Ana, and Oro Grande Range Complexes. Emergency Amb ulance Services include: advanced life support ambulance service; administrative transfers; transports to/from aero-medical evacuation sites; patient transports; and field ambulance services, as defined in Army Regulation (AR) 40-3, chapter 13, Emergency M edical Services. This AR can be found at www.asapa.army.mil. All contract personnel performing under this contract remain employees of the contractor and not employees of the Government. The Contractor must provide personnel who are competent, qualified a nd adequately trained to perform assigned duties. The Government will evaluate the quality of both the contractors professional service and their compliance with administrative documentation for purposes of contract inspection and acceptance. The list of contract line number (s) and items, quantities and units of measure are the following: 0001 Daily Service; 24/7 to include operational support 0002 Patient transport The contractor is allowed to make changes to the CLIN; however the government still reserves the right to structure the contract in accordance with Department of Defense FAR Supplement Subpart 204.71. FOB Destination shall be Fort Bliss, TX. 79916. Delivery of service will commence on 26 September 2007 with performance ending on 25 September 2008. The provisions at 52.212-1, Instructions to Offerors-Commercial Items applies to this combined acquisition The government plans to award the contract to the Lowest Price Technically Acceptable Offeror, considering Past Performance. The selected offeror must submit a completed copy of the provision at 52.212-3, Offeror Representation and Certifications  Commercial Items. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The following clauses in paragraph (a)(1) 52.233-3 and (b) of FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, will apply: 52.222-3, 52.222-21, 52.222-26, 52.232-33 and (c) 52.222-41.
