- It is the intent of this agency to award a sole source contract. Numbered Note 22 applies to this notice. This agency intends to award a purchase order to Phoenix Teledata Services, Inc. 370 Ranch Trail, Rockwell, TX 75032-5910 6 MDG requires the following: - Preventative Maintenance for Phoenix Teledata Hospital Telephone Appointing System (HTAS) to include telephone troubleshooting and technical assistance, software upgrades, script changes, and any necessary operational upgrades. QTY 12 MOS. A contract for HTAS Preventative Maintenance will be awarded to Phoenix Teledata Services, Inc. following no response back to this notice of intent. The purchase of HTAS Preventative Maintenance must be done through Phoenix Teledata Services, Inc. because they are the only responsible source and no other supplies or services will satisfy agency requirements. Market research was conducted for the purchase of HTAS Preventative Maintenance and Phoenix Teledata Services, Inc. is the only contractor found able to supply this service as we require. If no response to this notice is received by 4:00PM EST on Monday, September 17, 2007, the award for HTAS Preventative Maintenance will be made to Phoenix Teledata Services, Inc.
