- REVISED PRE-SOLICITATION NOTICE: The Department of Veterans Affairs (VA) on behalf of and as an agent of the Division of Strategic National Stockpile (DSNS) for the Centers for Disease Control and Prevention (CDC) will issue Request for Proposal (RFP) 797-DSNS-0714 for AMOXICILLIN TRIHYDRATE 500MG CAPSULES in bottles of 500 EACH. The period of performance will be as stated in the following delivery schedule: 1st Delivery no later than December 31, 2007; 28,644 Bottles of 500 EA 2nd Delivery no later than January 31, 2008; 28,644 Bottles of 500 EA 3rd Delivery no later than February 29, 2008; 28,644 Bottles of 500 EA 4th Delivery no later than March 31, 2008; 46,606 Bottles of 500 EA 5th Delivery no later than April 30, 2008; 46,606 Bottles of 500 EA 6th Delivery no later than May 31, 2008; 46,606 Bottles of 500 EA 7th Delivery no later than June 30, 2008; 46,605 Bottles of 500 EA Offeror's must state the exact name (name that appears on label) by which the drug will be supplied and have a unique NDC number. If the offeror is not a manufacturer, a signed letter of commitment between the offeror and the manufacturer must be submitted with the offer. As part of the award determination, the VA/DSNS reserves the right to consider additional package sizes if offered in a vendor's proposal. This will not affect the quantities listed in the usage requirements. Solicitation No 797-DSNS-0714 will be electronically issued in commercial item format in accordance with FAR Part 12 on or about September 19, 2007, with a closing date of October 4, 2007. The NAICS code for this procurement is 325412. Any future amendments and other miscellaneous documents will be available electronically for download at the following URL: http://www.fedbizopps.gov. No paper copies of the solicitation will be available and no telephone requests for paper copies of the solicitation will be accepted.
