- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being conducted under Simplified Acquisition Procedures (SAP) as a Request for Requisition No. F3Q4CA7054AH01. Award will be 100% SB Set-aside; only Small Businesses will be considered. NAICS Code:562211, business size NTE $11.5M; Place of Performance/FOB: Robins AFB, GA. Submit quotes for the following: Basic Period of Performance: 1 Year (from date of contract) and Four (4) One-year Options. Quotes shall contain prices for the option years. SERVICE REQUIREMENTS: Medical Waste Disposal Services in accordance with the attached SOW (Statement of Work). The purpose of this contract is to provide proper disposal of medical waste (i.e., needles, scalpels, and other medical waste). 1 SPECIFIC TASKS. 1.1 Contractor will pick up medical waste containers once a week from loading dock of Medical Treatment Facility (MTF). 1.1.1 Containers will include red bio hazardous bags, 18x22 boxes with maximum weight of 64 lbs and 18 gallon sharps containers 1.1.2 Pick-up should be scheduled in a.m. on a set day of the week. If the MTF is closed for any reason, the contractor will schedule pick up the following business day. 1.1.3 Contractor shall provide all boxes and red bio hazardous bags. 1.2 Contractor must be in compliance with all DHS, OSHA, and EPA regulations regarding the disposal of medical waste. 2 REFERENCES: 2.1 OSHA standard 1910.120. 2.2 All EPA regulations. 2.3 All DHS regulations. *** Amendment *** Expected Work load: between two (2) and six (6) boxes per day. During certain operations up to twelve (12) boxes may be needed. Contractor will be responsible for providing tracking labels with boxes.
