- The Department of Veterans Affairs VISN 23 intends to issue a solicitation for Prescription Eyeglasses under Request for Proposal (RFP) VA-263-07-RP-0022 for eligible veterans located in the jurisdiction of VISN 23 which includes the states of North Dakota, South Dakota, Minnesota, Iowa, Nebraska and selected counties in the states of Wisconsin and Wyoming. Delivery must be made in seven days or less. The successful contractor(s) must have fax capability. Period of performance is date of award through September 30, 2008 with four (4) one-year renewal options, subject to the availability of funds. This procurement is a HUBZone set-aside based on NAICS 339115 Opthalmic Goods Manufacturing, size standard is 500 employees. Proposal documents are available for download from the Fedbizopps web site @ www.fedbizopps.gov. Electronic offers will NOT be accepted. Solicitation response documents must show point of contact, address, and telephone number and fax number to be received in this office by October 08, 2007. Faxed proposals will NOT be accepted.
