Description
THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THE ATTACHMENT. This announcement constitutes the Request for Proposal and all solicitation documents are attached herein. A separate request for proposals will not be issued. The Court Services and Offender Supervision Agency for the District of Columbia is seeks contractors capable of providing Dually-Diagnosed Outpatient Substance Abuse Treatment Services The location where offerors proposed treatment facilities for the delivery of services must be located within (10) mile of the District of Columbia city limits. This notice is prepared in accordance with FAR Part 12 and FAR Part 5.2. THIS IS NOT A TOTAL SMALL BUSINESS SET-ASIDE PROCUREMENT. The NAICS code for this procurement is 621420. The solicitation number is CSOSA-07-R-0025. See ATTACHMENT for the following additional information: Instructions for completion of solicitation package, Schedule of Commercial Services and Prices, Statement of Work and the following clauses 52.212 1, Instructions to Offerors Commercial Items, 52.212 3 Offeror Representations and Certifications Commercial Items, 52.212 4 "Contract Terms and Conditions Commercial Items and 52.212 5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items. Offers or quotations will be evaluated in accordance with evaluation factors contained in the solicitation Evaluation Criteria. Questions regarding this Request for Proposal (RFP) must be submitted in writing via e-mail or fax no later than August 27, 2007 at 4:00 PM Eastern Standard Time (EST). You may email questions to bobby.jones@csosa.gov or fax them to the attention of Bobby Jones on (202) 220-5711. Offerors proposals must be received at the Government?s facility identified below by September 14, 2007 at 11:00 A.M. EST (LOCAL). Proposals received after this date and time will be considered late and will not be considered for award. The Government asks that proposals be valid for a period of up to 120 days after submission. Proposals shall be sent via mail, courier or hand delivered. Faxed or Email proposals will not be accepted. Proposals shall be delivered to The Court Services and Offender Supervision Agency, Office of Procurement, 633 Indiana Avenue, NW, Suite 880, Washington, D.C. 20004. You are hereby requested to submit your technical and cost proposals in accordance with sections 11.0 Instructions to Offerors and Section 12.0 Evaluation Criteria of the RFP. All proposals shall reference: CSOSA-07-R-0025 on the front of the package/page. The request for proposal document, incorporated provisions, and clauses are those in effect through Federal Acquisition Circular 2005-18.
