Combined Synopsis/Solicitation is amended to correct and modify previous published Solicitation Number RFQ VA-248-07-RQ-0236 This combined Synopsis/Solicitation supersedes previously issued Presolicitation Notice # VA-248-07-RQ-0191 posted on August 08, 2007.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The Bay Pines VA Healthcare System in Bay Pines, FL intends to purchase the following medical equipments: Symphony H&E Staining Instrument to provide Automated H&E slide preparation System, Reagents, Quality Control, and Technical Support as outlined below: Required instrumentation: 2 instruments for Histology Laboratory to include the following procedures for H&E slide preparation: Slide Identification with individual slide tracking technology, random access slide processing, Ability to designate slides and cases as STAT-on-demand, automated and on-board deparaffinization and Paraffin Removal, On-board and automated slide curing, Simultaneous processing of multiple staining protocols, Automated glass cover slipping with heat curing and the capability to immediately archive patient cases after processing. Instrument must be capable of deparaffinization, discrete slide staining and glass cover slipping, with one touch of a button. Complete one touch "walk away" automation; Xylene-free processing: Instrument must process, stain, deparaffinization and glass cover slip without the use of xylene. Instrument must utilize discrete slide staining - applying fresh reagent to each and every individual slide. Instrument must be completely module for additional redundancy and throughput. Instrument must allow for continuous slide loading and random slide processing. Instrument must allow for continuous access to reagents - ability to change out reagents while the system continues staining and cover slipping. The systems must have precautionary measures in place to eliminate human error and ensure correct use of slide processing and reagent loading. Specifically, the instrument must not allow the possibility of slides and/or reagents to be accidentally or incorrectly utilized. Instrument must have remote monitoring capabilities with the ability to detect and fix software problems remotely. Proactive remote diagnostic software capability maximizes system up time. Instrument must utilize a touch screen interface with software that allows for easily generated QA/QC reports. Instrument must have the capability to read barcodes and track individual patient slides through the system. Instrument must have the capability to prioritize cases with stat-on-demand functionality. STAT-on-demand must allow for quicker turn around time on curial cases, without negatively impacting the overall workflow or throughput. Instrument must utilize glass cover slips with a heat cured cover slipping processes that allow for instant adhesion and archiving of slides. Instrument must have a capacity to hold and completely process (from deparaffinization through cover slipping) 500 slides in 3 hours or less, with only one touch of button. Reagents must be prepackaged, pre-diluted and ready to use. Reagents must be sealed air tight and negatively pressurized until discretely dispensed onto the slide, so reagent oxidation can not occur. This ensures quality. Instrument must utilize RFID technology (Radio Frequency Identification) technology for Quality Control and proper tracking of reagent lot numbers, expiration dates, tests remaining, etc. The vendor will provide the following support services: Optimized reagents and disposables required for operating; technical support specialist to assist in equipment installation/set-up, staff training and methodology problems and questions. Technical support specialist shall assist in setting up the associated data management system and files. Pro-activity remote diagnostic monitoring (24 hours a day/7 days a week). The instrument must pro-actively monitor itself and then notify the user and manufacture if an issue arises. Any software problems need to be fixed and/or upgraded immediately via bi-directional internet connection, decreasing downtime and increasing productivity. Replacement parts and replacement modules need to be stored regionally to reduce downtime and increase productivity, should a hardware problem arise. 24/7 Call-in Technical Support Hotline A preventative maintenance schedule setup for vendors providing service. The delivery terms will be for all material to be delivered on or about 30 days after receipt of order; the preferred shipping method is FOB destination. Offeror shall indicate on their price quote if shipping method will be other than FOB Destination. The government anticipates awarding a single award for this requirement. The RFQ number VA-248-07-RQ-0236 shall be clearly identified on all submitted materials. The corresponding NAICS code is 334516; size standard is 500 the following provisions apply to this acquisition: 52.212-1 Instruction to Offerors; 52.212-2 Evaluation of Commercial Items. The specific evaluation criteria included in paragraph (a) are: 1) price 2) technical capability 3) past performance - past performance and technical capability when combined are equal in importance to price. Please submit 2 references for pass performance with your quote; all offerror's are cautioned that if selected for award, they must be registered with the Central Contracting Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov. Confirmation of CCR registration along with the following criteria: price; technical capability; and past performance including (compliance with the requirements of FAR Clause 52.211-6 Products) are a requirement. The following clauses also apply to this acquisition: 52.212-4 Contract Terms and Conditions. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-5-18.
