Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is set-aside for small business only. The NAICS Code for this synopsis/solicitation is 442210, Size Standard $6.5M. Solicitation/Purchase Request number F2KTAV7228A002 is issued as a Request for Quote (RFQ). This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-19 effective August 17, 2007. The Government intends to award contract line items for the purchase and installation of carpet. (CLIN 0001) Carpet Tiles in dark blue blend QUANTITY 361 sq yrds (CLIN 0002) Cove Base QUANTITY: 1 each (CLIN 0003) Install new carpet/Remove old (see statement of work dated Aug 14, 2007) QUANTITY: 6 each DESIRED DELIVERY DATE: September 28, 2007 FOB: DESTINATION. If you are not registered you may request an application at (888) 227-2423 or through the CCR website at http://www.ccr.gov. STATEMENT OF WORK Carpet Project SVF For WO# 30117 1. Description of Work The contractor shall furnish all parts, material, labor, equipment and supervision necessary to deliver and install carpet/rubberized floor tile. Site location: Kirtland AFB, NM, Kirtland Resource Mgmt Flight, located in Bldg 20320. 2. Major Activities (a) Scope of Work (1) Contractor shall deliver and provide own storage and safe keeping of any other supporting machinery, equipment, supplies, tools. (2) Contractor shall notify the authorized client representative Flight Chief of any electrical or other requirements that may adversely affect timely project completion prior to commencement of project. (3) Contractor shall be responsible for clean up and disposal of all refuse generated by project. (4) Contractor shall be responsible for any other action deemed necessary to insure completed project provides a finished end product that is fully functional and is in good working order. (b) Description (1) Remove/Dispose of old carpet that is currently in place. (2) Install carpet in the offices and conference room. (3) Install cove base in all areas where new carpet is being placed. (4) Carpet must be what was selected during the review process. (5) Perform site cleanup, removal of all materials and equipment (6) Participate in Project Inspection/Acceptance (7) Move Furniture 3. Key Project Collaborator(s) (a) The contractor shall provide the contracting officer and the Resource Manager Flight Chief a point of contact (POC). These individuals shall be designated in writing to include names and phone numbers. They shall have full authority to act for the contractor on all matters relating to the daily operation of this contract. (a) Security The contractor shall be responsible for obtaining a base entry pass in accordance with base entry requirements. This pass shall be used only to gain entry to the base to perform services as needed to fulfill all contractual requirements. This pass shall not be used to gain entry to any other area on base. (b) Safety The contractor shall follow industry safety standards, while assuring the site is secure from unauthorized access by the public. The contractor shall make every attempt to protect all Government and non-Government property and personnel on base from inconvenience and damage to property. 5. Project Schedule (a) Hours of Work: The contractor shall work with the golf manager to schedule hours of work. The preferred dates of project would like to be prior to 15 Sep 2007. (b) Project shall be completed within 30 days after notice to proceed.
