Description
Synopsis: The VA Great Lakes Acquisition Center intends to negotiate a sole source contract with Insight Instruments, Inc. to provide wide angle viewing systems for the Lecia 844 Microscope for the Jesse Brown VA Medical Center, 820 S. Damen Ave, Chicago, IL; Clement J. Zablocki VA Medical Center, 5000 W. National Ave., Milwaukee, WI; Wm. S. Middleton VA Hospital, 2500 Overlook Terrace, Madison, WI. To the best of our knowledge, Insight Instruments, Inc. is the only company that offers the SDI II electronic inverter that is compatible with existing equipment. This procurement is being conducted in accordance with FAR 6.302 - only one responsible source and no other supplies or services will satisfy agency requirements. Contract type is firm-fixed price and the NAICS Code is 423460. Equipment required at these locations allows the surgeon to comfortably and accurately see a wide view of the back of the eye during surgery. The stereoscopic diagonal inverter (SDI) helps to re-invert an inverted image. This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service, to the Contracting Officer within (10) calendar days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information must be sent to the Contracting Officer, VAMC GLAC, 5000 W. National Ave Bldg. 5, Milwaukee WI 53295 or via fax to (414) 902-5440. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source procurement with Insight Instruments, Inc.
