- Previous pre-solicitation synopsis dated 21 August 2007 is hereby revised to correct a term used for qualification of Home Visitors, from Accredited Clinical Social Worker (ACSW) to Licensed Accredited Social Worker (LASW). The synopsis is hereb y reissued in full. *** Army Contracting Agency, Directorate of Contracting, Fort Lewis, Washington is soliciting for proposals to award a single, firm fixed price type contract. The contractor shall provide all personnel and resources, except those specified as Government Furnis hed Property and Services, necessary to provide New Parent Support Program (NPSP) Home Visitor Services for Soldiers and Family Members at Fort Lewis, Washington. Five Home Visitors are required, one (1) of which shall be assigned as Lead Home Visitor to act as primary technical and administrative interface with the Governments Contracting Officers Representative and Lead Social Worker, and shall coordinate delivery of services by other contractor personnel. Contractor personnel shall possess and mainta in qualifications as follows: Lead Home Visitor shall, as a minimum, be a Licensed Independent Clinical Social Worker (LICSW) with 2 years of direct experience in child abuse and/or domestic abuse, or maternal and/or child health; and, all other Home Visi tors shall, as a minimum, be Licensed Accredited Social Worker (LASW) or Registered Nurse with 2 years of direct experience in child abuse and/or domestic abuse, or maternal and/or child health. The contract period of performance consists of a Base Period (1 November 2007 31 October 2008) and four one-year option periods. The proposed procurement is Unrestricted, and all qualified organizations are invited to submit proposals. The Request for Proposal (RFP) number is W911S8-07-R-0023. The applicable NAIC S code is 624190 and the small business size standard is $6.5 Million. A complete description and instructions for submission of offers for the New Parent Support Program Home Visitor Services will be provided within the performance work statement and will be posted to FedBizOpps and on the Fort Lewis Directorate of Contracting website. When issued, the solicitation will be available for downloading from our web site at http://www.lewis.army.mil/doc under Solicitations and FedBizOpps at http://www.fbo.gov after the estimated date of release. The estimated issue date is 5 September 2007 and the anticipated closing date is 5 October 2007 at 2:00:00 PM (PDT). All responsible sources may submit an offer which will be considered. Firms offering a response to thi s notice should ensure they are registered in the Central Contractor Registration Database at http://www.ccr.gov/ and the applicable NAICS code 624190 is included in the CCR profile prior to submission of offers for consideration.
