AVAITATION BLVD., EL SEGUNDO, CA 90245-2808 Description This is a solicitation for the following commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. All responses to this Solicitation/Request for Proposal (RFP) must make reference to Solicitation Number FA2816-07-R-0015. This acquisition is set-aside for Small Businesses. The North American Industry Code (NAIC) associated with this requirement is 621399, Medical Management Nurse (UM/CM/DM), with a small business size of $8.5mil. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-17. Los Angeles Air Force Base has a requirement for 1.5 Full Time Equivalent (FTE) (one full time and one part-time, 20 hours/week) Registered Nurse (RN) positions for this Performance Work Statement (PWS). The Health Care Staffing Firm (HCSF) (i.e., the Contractor) shall provide Medical Management services required for Government TRICARE enrolled beneficiaries. The Contractor shall provide care at the 61st Medical Group (61 MDG), also referred to as the "Medical Treatment Facility" (MTF) herein. Performance shall be according to the requirements contained in the PWS in accordance with the Department of Defense (DoD) Medical Management Guide and MDGI 40-6, Medical Management Plan. Professional organizations such as the Utilization Review Accreditation Commission (URAC), the Case Management Society of America (CMSA), and the Disease Management Association of America (DMAA) provide industry standards and guidelines for the practice of specific components of Medical Management services. The Contractor shall perform services in accordance with the ethical, professional, and technical standards of the health care industry and DoD guidance. The 61 MDG operates two geographically separated Primary Care Clinics and an Aerospace Medicine Clinic. The main clinic is located in El Segundo, CA and the Ft. MacArthur Satellite Clinic is located in San Pedro, CA. The Contractor shall provide Medical Management services to support clinic operations at each geographic location. When reference is made to MTF, services are inclusive of clinic locations in El Segundo and San Pedro, CA. Medical Management services shall be provided to 61 MDG TRICARE enrolled beneficiaries who reside within a 50 mile radius of the Medical Treatment facility. Services may also be required for other non-enrolled categories of beneficiaries, such as TRICARE Plus or Standard beneficiaries. Services provided will be prioritized in accordance with the 61 MDG Medical Management Plan. The Performance Work Statement (PWS) is attached to this posting. The proposed contract shall be for a one 12 month Base period (21 Sep 2007 ? 20 Sep 2008) with four one year option periods.
