Description
NOTIFICATION OF INTENT TO AWARD A SOLE SOURCE PURCHASE ORDER for The Department of Health and Human Services (DHHS), Indian Health Service (IHS), Office of Clinical and Prevention Services (OCPS) to the Family Violence Prevention Fund (FVPF). The purpose of this sole source performance based contract is to support the mission of the Indian Health Service (IHS) to raise the health of the American Indian and Alaska Native (AI/AN) people to the highest level by providing the following services: To improve health care delivery and health outcomes for AI/AN health care users. Fourteen sites are participating in the Chronic Care Initiative. Three of the fourteen sites are Domestic Violence Pilot sites and are screening women for DV. Eleven of the sites need additional support to implement DV screening within the context of a proactive, planned care delivery system, not as a stand alone ?silo?d? screening and patient education intervention. The Contractor shall provide materials, TA, and will make two presentations at planned CCI meetings on DV screening, documentation, and institutional DV reform. DV content and materials will be tailored so that it fits into the population based care approach. At a minimum, DV background information will be incorporated into an overall screening session and a decision support session. Interested parties possessing these required capabilities should submit a cover letter, along with their capability statement, demonstrating the ability to perform all aspects of this requirement. Responses must include: the contractor?s name and business address; the name, phone number and e-mail address of a point of contact; the contractor?s Tax Identification Number and DUNS number (so the Government can verify that the firm is registered in the CCR); a list of previous contracts for similar type requirements; and any company literature or brochures that may be relevant. This notice of intent is not a request for competitive proposals. However, all capability statements or proposals received within 15 days after the date of publication of this synopsis will be considered by the Government. Small business concerns (including small businesses, small disadvantaged businesses, HUBZone small businesses, woman-owned small businesses, veteran-owned small businesses, etc.) are encouraged to review this notice. A determination by the Government not to compete the proposed acquisition is based upon responses to this notice and is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Any requests for solicitation packages will be disregarded.
