Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W91YTV-07-T-7707. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18. Federal Acquisition Regulation (FAR) Clause 52.211-6, Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. The North Americ an Industry Classification System Code (NAICS) for this procurement is: 423450; the Standard Industrial Classification Code is 5047; and the small business size standard is 100 . This requirement is 100% Small Business Set Aside. The U.S. Army Aeromed ical Center, Fort Rucker, Alabama, has a requirement for the following: Item No. 0001: One (01) each RM-184 Acoustic Systems Group Test Booth, 8 Test Positions, Single Wall. Specs: Isolated acoustical floor; carpeting with cove base trim; sealed 32 clear opening acoustical door with camlift hinges; double glazed, laminated, acoustical, safety glass, 24 wide by 18 high observation window in door; medical Jack panel at each test position for left and right earphones, patient response switch corresponding to master panel; test position divider curtains; padded seating for eac h test position; electrical service with 20 ft. power connection; recessed incandescent lighting with wall switch (4 light fixtures); acoustically engineered ventilation mounted external to walls or roof panels (400 CFM; inside (W x L) 74 x 100; outsid e (w x L) 80 x 108 UNIT PRICE $__________, EXTENDED PRICE $__________. Item No. 0002: One (01) each, Installation at Fort Rucker, Alabama, UNIT PRICE $__________, EXTENDED PRICE $__________. Item No. 0003: One (01) each, Freight Charges to Fort Rucker, Alabama, UNIT PRICE $__________, EXTENDED PRICE $__________. Quote all or none. Quote Brand name or equal. If submitting a quote for a substitute item, please provide full specifications to allow technical evaluation by the end-user. The delivery terms will be for all material to be delivered on or about 30 days after receipt of order. The preffered shipping method is FOB destination. Offeror shall indicate on their price quote if shipping method will be other than FOB Destination. The government anticipates awarding a single award for this requirement.
