Description
The U.S. Army Corps of Engineers, Jacksonville District, is seeking employee assistance services for permanents federal employees located in Jacksonville, field offices through out Florida and Puerto Rico. The purpose of the Employee Assistance Progr am (EAP) is to provide employees and family members a broad range of counseling services and other assistance for any personal or work-related conflict or concern. The range of concerns to be covered includes any emotional, behavioral, mental or personal c oncern, especially those related to alcohol or drug abuse but not limited to, which may adversely impact on the employees job performance, attendance, or conduct. The EAP is available to all permanent Jacksonville District employees and their immediate fa mily members (immediate family member as defined under health insurance coverage), upon request. Services will cover a total of approximately 874 employees with 566 located at the Jacksonville Headquarters office, 268 in various field offices, and 40 emplo yees in Puerto Rico. A list of offices and number of permanent employees will be provided with the solicitation. The requirements of the Performance Based Statement of Work will include but not limited to: 1) Direct, short-term, in-person counseling, on or off-site, to employees and immediate family members for evaluation of problems and/or concerns; 2) Educational and/or outreach activities, as requested; 3) Employee orientation to counseling program; 4) EAP program training; 5) Acceptance of management or self-referrals; 6) Case file maintenance, quarterly and yearly statistical reports, and other progress reports as needed, and program evaluation; 7) Confidential Record Keeping System. A firm-fixed price contract is anticipated with a base period of one y ear from the date of the award with option to extend for four additional one-year periods. Services will be procured pursuant to FAR Subpart 13.5 Test Program for Certain Commercial Items. The Government will award a contract resulting from this solicitati on to the responsible offeror whose quote conforming to this solicitation will be most advantageous to the Government, price and other factors considered. The following BEST VALUE factors in the order of their importance shall be used to evaluate offers: ( 1) Technical, (2) Past Performance, (3) Price. All responsible sources may submit quotes, which, if timely received, the Government shall consider. This requirement is set aside 100% for small business. The NAICS code is 621330 with a size standard of 6.5 million. The solicitation will be issued on or about 6 September 2007, with a due date on or about 20 September 2007. Solicitation will be issued in electronic format only.
