Description
1. The 10th Mission Support Group, Directorate of Contracting, United States Air Force Academy Colorado, intends to award a Brand Name Only, Blanket Purchase Agreement (BPA), to Metro Medical Equipment and Supply Inc., 7550 N. Broadway, St. Louis, MO 63147. The BPA is for Smith and Nephew brand surgical supplies, surgical instruments and patient implantable items. The items are for use with a Smith and Nephew video tower and Information Management System (IMS). Smith and Nephew is the manufacturer of the products and Metro Medical Equipment and Supply Inc. is the only known source and distributor for the Smith and Nephew products. Use of other than Smith and Nephew brand products with the Smith and Nephew video tower and IMS creates a safety and equipment risk because use has the potential of causing damage to the Smith and Nephew surgical equipment creating an unacceptable risk for patients. Additionally, the surgical staff requires that a Smith and Nephew technical representative be on call 24 hours/7 days a week in order to provide the necessary consultation to operating room personnel. The period of performance for this BPA is five years. 2. The North American Industry Classification System Code [NAICS] is 423450 and the size standard is 100 employees. 3. This synopsis is for notification purposes only and does not constitute a solicitation for bids or proposals. Any company that is capable of providing the Smith and Nephew brand requirements shall notify John Kurko, Contract Specialist, in writing not later than 1200 hours, Mountain Time Zone, 28 August 2007. The written response shall include statements to support their claim. The government does not intend to pay for any information provided under this synopsis. 4. All questions should be referred to John Kurko, Contract Specialist, 719 333-3948 or email to john.kurko@usafa.af.mil. 5. 5352.201-9101 OMBUDSMAN (AUG 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
