- Description
- The National Hansen?s Disease Programs (NHDP) has a requirement for physical therapy consultant services. The services include assessment of patients, identification of therapy diagnosis, formulation of therapy treatment plan and goals and implementation, supervision and modification of therapy program for each patient. The focus of treatment shall be on patients with neuropathic limb dysfunction secondary to Hansen?s Disease, performance of Musculoskeletal and ADL services, assist NHDP staff in training of other healthcare professionals in the treatment of HD and provide research support related to studies involving improvements in the care and treatment of neuropathic limbs. Services are required up to 40 hours per week from date of award through September 30, 2007 with 3 one-year options. This is a 100% small business set-aside, restricted to small businesses eligible to receive award. The RFP will be posted at the Government website at www.fbo.gov under HHS, Health Resources and Services Administration, BPHC/National Hansen's Disease Programs, Posted Dates on or about July 2, 2007. It is the offeror's responsibility to monitor the site for amendments to the solicitation. The NAICS code for this requirement is 621340. Solicitation will be conducted under Federal Acquisition Regulation (FAR) Part 12 and 13. The closing date for receipt of proposals will be approximately thirty (30) days after issuance/posting of the RFP.
