- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81NTE7208850 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition C ircular 2005-18. This acquisition is solicited on an unrestricted basis for NAICS 334510, size standard 500 employees. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand Name or Equal applies to this RFQ. Item no. 0001: ProFractional Module Upgrade, Sciton part no. 1500-477-90 or equal, 1 each. Salient characteristics: Designed to comfortable treat scars on face and body, acne scars and improve skin tone and texture of the dermis by heating a defined area of tissue. The ProFractional creates micro-ablation channels 250 microns in diameter and leaves tissue untreated around channel for faster healing. Item no. 0002: Three Multispot Small Adaptors, 1 set. Salient characteristics: Set consists of one of each: small 15mm x 15mm adaptor for use on small areas of the face such as foreheads, Sciton part no. 1500-202-18 or equal; 11 mm round adaptor used to target pigments and A/Ks, Sciton part no. 1500-22-25 or equal; 7 mm round adaptor use to target small vascular issues, Sciton part no. 1500-202-24 or equal. Item no. 0003: BBL-X Extended Crystal BBL Handpiece, Sciton part no. 1500-202-26 or equal. Salient characteristics: Enables Multispot Adaptors to snap on to handpiece making treating small vessels, pigments, and lesions easier to target. Required Delivery: On or before 30 September 2007 to Brooke Army Medical Center, 3851 Roger Brooke Dr, Fort Sam Houston TX 78234. FOB: Destination. Vendors shall comply with FAR 52.211-6 if submitting other than the brand name products. Evaluation of Quotes: The Government will award a contract resulting from this solicitation to the responsible vendor whose quote, conforming to this solicitation, is Lowest Priced, Technically Acceptable (LPTA). Award will be on an all or none basis. T he following factors will be used to evaluate quotes: 1. Technical Acceptability: In order to be considered technically acceptable, vendors must demonstrate that all items proposed [if other than Brand Name] meet or exceed the salient characteristics lis ted above. 2. Price. All quotes will be evaluated on Technical Acceptability. The award decision will be based on the Lowest Priced, Technically Acceptable quote.
