- SOURCES SOUGHT. This is a sources sought announcement only! This is not a solicitation announcement. The purpose of this annoucement is to gain knowledge of potential qualified sources such as 8(a), HubZone, small disadvantage, service disabled veteran-owned business, and small business relative to NAICS 621111, size standard $9M. Responses will be used to determine appropriate acquisition decisions. The Department of Veterans Affairs, Carl Vinson VAMC, 1826 Veterans Highway, Dublin, GA is seeking sources for a potential contract to provide the establishment of a Community Based Outpatient Clinic (CBOC) in Macon, Georgia. The contractor is to provide the continuous delivery and management of primary care for all assigned VA patients at their establishment. The contractor will provide health care providers, medical facilities, medical equipment and supplies, and all administrative functions to fulfill the care and range of services as required by the VA and Joint Commission on Accrediation of Healthcare Organizations (JCAHO), or other regulatory agencies. (NOTE: Contractor's facility must be physically located with the following zip codes: 31204, 31206, 31210, 31217, 31220, 31211. Contractor is responsible for ensuring that all persons, whether they be employees, agents, sub-contractor's, providers or anyone acting for or on behalf of the contractor, are properly licensed at all times under Georgia state law and/or regulations and shall be subject to credential and privilege approvals by the VA. The intended contract is anticipated as one base year, with 4 option years. Responses to this sought must include 1. Point of contact. 2. Company Name, 3. Address, 4. Telephone & Fax numbers, 5.
