Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation VA-69D-07-RQ-0400 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-18. No telephone offers will be accepted. Only written offers received directly from the requester are acceptable. If submitting by FAX, you may send your offer to 414-902-5440, ATTN: Kevin Baier. The North American Industry Classification System (NAICS) Code 334510. This size standard is 500 employees. The proposed solicitation listed is a 100% Total Small Business set-aside. Contractors are encouraged to submit offers accordingly. Item 0001: Steridox VP Chlorine Dioxide Sterilizer: Model Number: SSR-2A-MCV, QTY 1: Must have the following features: 1. Stainless Steel Interior: 20" W x 20" H x 39" D 2. Sterilant Gas Concentration Monitoring and Control 3. One 316 Stainless Steel Radial Arm Door 4. PLC Process Control, Variable Cycle Software 5. Includes Concentration, Relative Humidity, Temperature, and Pressure Sensors 6. Cabinet Style Enclosure (no recessed flanges) 7. Utilizes house steam 8. Includes 3 days on-site startup support 9. Includes Installation ( customer needs to have exhaust, drain for liquid vacuum pump, city water feed, 3 phase 240VAC power to sterilizer). 10. To include 3 CD Cartridge sets, Model # 10-001 11. 3 year warrantee (parts and Labor) 12. Removal of Old EtO Sterilizer and EtO Abatement System. Delivery is to be FOB Destination to 5000 W. Incorporated herein by reference the following FAR Clauses and Provisions apply to this acquisition: 52.204-9, Personal identity Verification of Contractor personnel (NOV 2006); 52.211-6, Name Brand or Equal; 52.212-1, Instructions to Offerors - Commercial Items (SEP 2006) - offerors must comply with all instructions contained in this provision; 52.212-2, Evaluation - Commercial Item - The Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technically acceptable (2) Price and (3) Delivery. Technically acceptable is more important than price and delivery. To be technically acceptable the item shall meet the Government's specifications. Offerors shall submit descriptive literature, product samples if applicable, technical features and warranty provisions. The contractor agrees to furnish one copy of a manual, handbook or brochure containing operating, installation, and maintenance instructions (including pictures or illustrations, schematics, and complete repair/test guides as necessary). Where applicable, it will include electrical data and connection diagrams for all utilities. The instructions shall also contain a complete list of all replaceable parts showing part number, name and quantity required; 852-211-71, Guarantee (NOV 1984) - The contractor guarantees the equipment against defective material, workmanship and performance for a period of said guarantee to from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material which are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside the continental United States.
