- Description
- Provide equipment and technician for Mobile Lithotripsy Services at the Department of Veterans Affairs Medical Center (VAMC), Alexandria, LA. Services shall be provided 1 or 2 days per month, depending on need. The contractor will provide a trained, licensed, experienced technician to assist a VA physician in the lithotripsy process. Lithotripsy services shall be provided by a mobile unit at VAMC Alexandria, 2495 Shreveport Highway, Pineville, LA 71360-4044. The procurement provides non-invasive therapy for kidney stones to alleviate the need to refer veterans to other medical centers for stone removal. The VAMC Alexandria is contemplating a firm-fixed price contract. The proposed contract will have a twelve-month base period, on or about 5/1/2007 through 4/30/2008, with four optional one-year provisions. The NAICS code is 621999/size standard $9M applies. The VA will award a contract resulting from the Request for Proposal (RFP) to the responsible offeror whose offer conforming to the RFP will be most advantageous to the Government. The following factors will be used to evaluate quotes: Price and Past Performance. Copy of the RFP will be made available not earlier than 3/16/2007 via FedBizOpps web site, http://www2.fbo.gov/spg/VA/index.html under VA Alexandria. Any amendments issued to this solicitation will be available at this web address. Offerors are advised that they are responsible for obtaining amendments, it is essential that offerors register on bidder's mailing list for this solicitation to receive notification of amendments.
