- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. This is a solicitation of Purchase Request FM500071980066 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-18. The associated North American Industrial Classification System (NAICS) code for this procurement is 334515 with a small business size standard of 500 employees. This acquisition is set aside for small businesses. All responsible Contractors shall provide a quote for the following: Line Item 0001: Air Caloric Stimulator Package, to include: ICS Chartr 200 VNG/ENG 4-ch with computer & printer, P/N 8-04-11904; Quantity: 1 Ea NCA-200 Air Caloric Stimulator with digital display features an otoscopic head, illumination of the point of delivery an built-in air supply, P/N 8040012; Quantity 1 Ea 2-shelf cart, P/N MC-148-48; Quantity 1 Ea Freight to Elmendorf AFB Delivery shall be completed not later than 30 days after receipt of an order and shall be FOB Destination.
