Description
The purpose of this amendment is to answer the following questions: 1. "USU will provide the Death Call phone number and the contractor will maintain that number." Questions: QUESTION: (a) What/whose phone number will USU donor families be advised to call? ANSWER: USU will be called with notification of death, as published in the USU Anatomical Gift Program (AGP) items (i.e., wallet card, donation forms). The number is located at USU. QUESTION: (b) How is that line to be answered at night? ANSWER: After duty hours, the AGP phone number is transferred to USU Security for notification purposes. Once a death call is received, USU Security will then notify the Contract Funeral Service of the death. The Contract Funeral Service must notify the USU AGP of the death call the next duty day. QUESTION: (c) Who will be responsible for the paying phone bill for that number? ANSWER: The number is located at USU. USU will be responsible for paying the phone bill. QUESTION: (d) Who will be responsible for paying an answering service to answer that number at night? ANSWER. The number is located at USU. USU will be responsible for answering the phone. 2. Performance and Delivery, Paragraph (b): "... refrigeration for an additional period not to exceed 72 hours." QUESTION: What if USU is closed for more than 72 hours (e.g. on holiday weekends)? ANSWER: Normally a body should be held no more than 72 hours in refrigeration prior to transfer to USU. At times, there will be extenuating circumstances in which the remains must be held for a longer period. Prior to delivering any remains held more than 72 hours the Contract Funeral Service must receive written authorization from the Contracting Officer?s Representative. 3. Permits, Paragraph 1: "... contractor will furnish all necessary permits at no additional cost to USU." QUESTION: Doesn't this wording contradict the listed "Service 0001AQ: Procurement of D.C. Medical Examiner's approval for anatomical donation and/or cremation"? Additional stops/trips are required to obtain ME permits in D.C. and Virginia as well as transit permits in Virginia. The contractor should be permitted to charge for any such additional trip/stop, as required by the circumstances. ANSWER: Clin 0001AQ description is amended to read as: Procurement of D.C. Medical Examiner?s fee for anatomical donation and/or cremation A Clin will be added for any additional trip/stops. 4. Other Services/Fees: 0001AK: Transfer . over 51 miles radius .: Questions: QUESTION: (a) What is the maximum distance the contractor will be expected to travel for a transfer? ANSWER: The maximum distance is 150 miles. The contractor can only bill up to 4,000 miles for the year. QUESTION: (b) Above a distance of 50 miles, it may necessary to engage a local funeral home to pick up a body (e.g. from a residence or a nursing home) and refrigerate the body until the contractor can get there to pick it up. May our bid specify that in such cases we will add the other funeral home's charges to ours? ANSWER: If necessary, the contractor is authorized to engage a local funeral service to provide the professional services as required. If so, the contractor is responsible for the performance of the local funeral service. 5. QUESTION: 0001AR: For clarity, can you add the words "Procurement of" at the beginning? Clin 0001AR description is amended to read as: Procurement of VA Medical Examiner?s fee for donation and/or cremation 6. QUESTION: 0001AT: Death Certificates: The family has always paid for the filing of the death certificates and certified copies of the death certificate. Is USU now going to pay for the filing and certified copies for the family? ANSWER: CLIN 0001AT will be deleted. 7. QUESTION: 0001AV: After Hour Death Call, including obtainment of vital statistics: Can you amend the solicit NOTE: THIS NOTICE MAY HAVE POSTED ON FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (06-JUL-2007).
